The major obstacle to the treatment of pathologic pain is our limited ability to selectively activate circuits in the central nervous system while minimizing activation of circuits that innervate the site of pain. This problem is significant to many pathologic pain syndromes, including some of the chronic pain seen in patients with rheumatoid arthritis and cancer. Recent studies with deep brain stimulation (DBS) and optogenetics suggest that inhibition of ensembles of neurons is a promising treatment for pathologic pain. However, there are critical issues for which no clear guidance exists. First, it is unknown how to induce inhibitory activation of large groups of neurons in a manner that is effective in the face of natural variability and inhomogeneities in the network dynamics. Second, it is not clear to what extent the energy requirements of such inhibition will severely limit DBS implant longevity. Third, it is also not clear whether chronic inhibition will lead to desensitization to inhibitory control, or immune system activation, or both. Finally, it is currently unknown whether it is appropriate to target inhibitory circuits within the thalamus in order to achieve pain relief, and whether such inhibition of the thalamus might limit activity in thalamic regions that play a critical role in generating and inhibiting pain.
